To access Ontario HealthatHome In-Home Stroke Programming, patients would need a referral to Ontario Health atHome, which can be found here: Making a Referral | Ontario Health atHome Once referrals are received, our intake department will call patients to complete an eligibility assessment. If it is determined based on this assessment that patients can access the therapy services they need through other available community programs (i.e. hospital led programs, outpatient programs, community programming), patients will be directed there. But if all other appropriate and available resources are exhausted and patients require in home rehabilitative care, then they are onboarded for services. Our in-home stroke program is meant to target patients who are acutely post stroke (traditionally right following hospital discharge), but we have supported patients up to one year following their stroke incident. The program here can include any combination of OT, PT and SLP services, based on patient’s assessed needs and goals.
